- I agree to abide by PHS Policy, USDA Regulations, the Guide for the Care and Use of Laboratory Animals, the Guide for the Care and Use of Agricultural Animals in Agricultural Research and Teaching, and all relevant institutional, state, and federal regulations and policies regarding animal care and use at The Ohio State University.
- I will promptly notify the IACUC and the clinical veterinarian regarding any serious adverse events and unanticipated problems that impact the well-being of the animals involved in this work. Furthermore, I will permit emergency veterinary intervention for any animals.
- I declare that all individuals having direct live animal contact, including myself, have completed the institutionally required "Animal Usage Orientation Course" and "Occupational Health and Safety Training," are trained in acceptable and humane procedures for the handling, care, and use of the animal species involved in this work, and are aware of the hazards involving the use of live animals and tissues.
- I declare that all individuals involved in this work, including myself, are qualified to conduct procedures involving animals, are competent in the techniques cited in this protocol, and will maintain appropriate and complete animal records. I ensure that copies of the approved protocol will be made available to all personnel involved.
- I assure that all individuals, including myself, are participating in the institution's Occupational Health and Safety Program through the completion of a current Occupational Health and Safety Risk Assessment.
- Should I use the work described in this protocol for a proposal for funding, I assure the description of the animal use in such funding proposal to be identical to that contained within this protocol.
- I understand that the use of hazardous agents may only be initiated after approval from the IACUC, Institutional Biosafety Committee (IBC), the Radiation Safety Committee, and/or Environmental Health and Safety, as appropriate.
- I assure that an "Animal Use Protocol Annual Review" will be completed and submitted prior to the annual anniversary of protocol approval to avoid expiration of such approval and cessation of all research activities.
Initiation and Animal Use
- I assure that the work described within this protocol will not be initiated until notification of IACUC approval has been received.
- I assure that I will notify the IACUC of any proposed changes regarding the work described within this protocol and agree that no such changes will be implemented until approved by the IACUC (except where absolutely necessary to eliminate apparent immediate hazards to individuals and/or animals).
